Why all the Paperwork?
Filling out paperwork is a pain but these documents protect the event organizers, the filmmakers and the talent. The risk is very small but possible on small scale productions. But if there’s any chance your short could be sent to a distributor, festival or other contest, releases will be required.
And of course, in the event of a lawsuit, a release is extremely helpful.
Obtaining proper written permissions is the responsibility of the producer. Get them filled out and make copies for your records!
There is no need for a video release form if a person in a video cannot be identified. Shooting a crowd of people in a public place may also not require a release unless it appears that the people were advocates for a company or business.
Getting release forms signed by identifiable participants in your film is a good idea not only to protect yourself, but to give you options for selling the rights to your film in any kind of commercial venture or competition. It’s just a good habit to develop.
All paperwork is due at submission unless we are notified, in which case it must be submitted no later that one week after the submission deadline.

What do I do with all this Paperwork?
Keep it organized and make copies for your own records. Submitted forms will not be returned.
Your paperwork can be scanned and uploaded with your film or it can be emailed to us at email@example.com.
All forms and releases must be turned in within a week of submission of your work. It’s best to complete forms as you go and submit them together with your film. Cast and crew releases must match End Credits.

Team Leader Agreement - 1 per Team - Due at or before Kick-off
The Team Leader form indicating agreement to the rules and requirements stated on this site.
This form is due at or before Criteria Kick-off on Friday, September 14, 2018.
Team Member Roster - 1 per Team - Due at Sumbmission
Use this form to make sure you have everyone’s Waiver/Releases and help with end credits!
Fill in names, jobs and email, and make it readable.
Cast and Crew Agreement Form - 1 per Team Member - Due at Submission
This is a combined talent release and liability waiver. This for protects the team leader, as well as the film and HHM Indemnities.
- All cast and crew must sign
- Each team member must sign a separate form.
- Minors need their own form and it must be signed by a legal guardian.
Location Agreement - 1 per location - Due at Submission
This Location Release Form is due at submission but will be accepted up to a week after submission with notification.
This must be signed by a manager if the owner isn’t available.
Permits are not required for submission.
Music Release Form - 1 per artist or song - Due at Submission
Every song must be listed.
An artist or composer may list multiple songs if they are solely written and produced by the artist.
Materials Release Form - 1 per item/creator - Due at Submission
Used for everything else used and/or created before the challenge dates but within rule guidelines, such as sound effects, photographs, stock footage (see rules), post production effects or filters and any other materials.
Multiple materials may be on one form if produced by the same creator.
Submission Form and Creative Certification - 1 per Team - Due at Submission
This form should be filled out after your film is FINAL. This is the boss of all HHM Film Slam forms.
- It is a great way to quadruple check that you have met all requirements.
- It gives judges valuable insight on your film.
- It gives you slate requirements so you don’t have to look them up online.
- It helps with the render process.
- It certifies you’ve completed all work during the Official Time Period.
- It invites you to give us feedback.

The Legal Stuff – Our Film Terms of Submission Notification
HHM Film Slam, Hell’s Half Mile Film and Music Festival, Hell’s Half Mile Events affiliates, officers, directors, agents, co-branders or other partners, and any of their employees or volunteers are referenced below collectively as HHM Collective.
By submitting an Entry into this HHM Film Slam, each Entrant warrants and represents that the Entrant owns all rights to the Entry he/she is entering in this HHM Film Slam event, including, without limitation, the video or digital recording, and the performance contained in each Entry, with the exception of any assets provided by the HHM Film Slam for use by Entrants in connection with this HHM Film Slam event. Each Entrant further warrants and represents that the Entrant has obtained permission from each person who appears in the Entry to grant the rights to the HHM Film Slam parties described in these Official Rules and will submit copies of such permissions to the HHM Film Slam parties. By submitting an Entry, Entrant agrees to the Official Rules, and further agrees to indemnify and hold the HHM Film Slam and HHM Collective, harmless from any and all claims, damages, expenses, costs (including reasonable attorneys’ fees) and liabilities (including settlements), brought or asserted by any third party against any of the HHM Collective due to or arising out of the Entrant’s Entry materials in this challenge event, or the Entrant’s conduct during and in connection with this HHM Film Slam event, including but not limited to trademark, copyright, or other intellectual property rights, right of publicity, right of privacy or defamation. Entrant agrees to release, indemnify and hold harmless HHM Collective from any and all claims that any advertising, presentation, web content or any other material subsequently produced, presented, and/or prepared by or on behalf of HHM Film Slam or the HHM Film Slam event, infringes on the rights of Entrant’s work as contained in any Entry.
By participating, Entrants agree to these Official Rules which are final and binding in all respects. The HHM Film Slam is governed by the laws of Michigan and all claims must be resolved in the Michigan courts.
More Legal Stuff – Our Limitations of Liability and Release Notification
No liability or responsibility is assumed by HHM Film Slam, Hell’s Half Mile Film and Music Festival, Hell’s Half Mile Events or HHM Collective parties resulting from any user’s participation in or attempt to participate in the HHM Film Slam or ability or inability to upload or download any information in connection with participating in the HHM Film Slam event. No responsibility or liability is assumed by the HHM Film Slam or HHM Collective for technical problems or technical malfunction arising in connection with any of the following occurrences which may affect the operation of the HHM Film Slam event: hardware or software errors; faulty computer, telephone, cable, satellite, network, electronic, wireless or Internet connectivity or other online communication problems; errors or limitations of any Internet service providers, servers, hosts or providers; garbled, jumbled or faulty data transmissions; failure of any e-mail transmissions to be sent or received; lost, late, delayed or intercepted e-mail transmissions; inaccessibility of the website in whole or in part for any reason; traffic congestion on the Internet or the website; unauthorized human or non-human intervention of the operation of the HHM Film Slam event, including without limitation, unauthorized tampering, hacking, theft, virus, bugs, worms; or destruction of any aspect of the HHM Film Slam event, or loss, miscount, misdirection, inaccessibility or unavailability of an email account used in connection with the HHM Film Slam event. HHM Film Slam and HHM Collective are not responsible for any typographical errors in the announcement of prizes or these Official Rules, or any inaccurate or incorrect data contained on the website or social media sites maintained by the HHM Film Slam and their affiliates. Use of website and the HHM Film Slam’s social media sites is at user’s own risk. HHM Film Slam and HHM Collective are not responsible for any personal injury or property damage or losses of any kind which may be sustained to user’s or any other person’s computer equipment resulting from participation in the HHM Film Slam event, use of the website or the download of any information from the website.
